Adult Patient Intake Form
For new adult patients — includes complete medical history, current symptoms, insurance information, and your personal health goals.
Personal Injury Form
For patients involved in auto accidents, workplace injuries, or personal injury cases. Includes accident details, insurance, and attorney information sections.
Children's Intake Form
Designed for pediatric patients ages 1–17. Includes parent/guardian information, developmental history, birth information, and childhood health concerns.
Infant Form
For newborns and infants under 12 months. Covers birth history, birth method, any birth complications, developmental milestones, and parental health concerns.

Complete Forms
Download and fill in your intake form before arriving, or complete it in our welcoming waiting room.
Meet Dr. Maxwell
Sit down with Dr. Maxwell for a thorough consultation. He'll listen to your concerns, goals, and health history.
Examination
Comprehensive spinal examination and postural analysis. X-rays may be taken if clinically indicated.
Your Care Plan
Dr. Maxwell presents your findings and outlines a personalized care plan tailored to your goals and lifestyle.
How Long is the First Visit?
Plan for approximately 45–60 minutes for your first appointment. Follow-up visits are typically 20–30 minutes.
What to Wear
Comfortable, loose-fitting clothing is ideal. We may ask you to change into a gown for certain parts of the examination.
What to Bring
Your completed intake forms, photo ID, insurance card, and any relevant medical records or imaging (X-rays, MRIs).
